Frozen Shoulder Management

  • Frozen shoulder aka adhesive capsulitis is a painful condition of the shoulder resulting in the restricted or no movement of the shoulder joint. It is caused by immobility or reduced mobility of the shoulder joint due to trauma, illness, inflammation or post-surgery immobility.
  • Frozen shoulder is treated by:
    • Medications like NSAIDs and steroids.
    • Physical therapy to recover the mobility of the shoulder joint.
    • Steroid injections to decrease pain and improve shoulder mobility.
    • Joint distension by injecting sterile water into the capsule of the shoulder joint to stretch the tissue and improve mobility.
    • Transcutaneous electrical nerve stimulation (TENS) uses electric current to stimulate the release of pain inhibiting molecules called endorphins or block pain impulses.
    • Shoulder manipulation is performed under general anaesthesia and the shoulder joint is moved in all directions by the surgeon to lose the tightened tissue.
    • Surgery for frozen shoulder is performed rarely and advised in cases of frozen shoulder showing no improvement by therapy or medications. It is also called arthroscopic surgery.
      • The surgery is performed under general anaesthesia with the use of an arthroscope ( a thin tube fitted with a light and tiny camera) and other instruments. These are inserted into the joint through small cuts and are used to remove scar tissue and adhesions to loosen the shoulder joint.

ACL Reconstruction Surgery

  • The anterior cruciate ligament is a ligament that crosses the middle of the knee connecting the thigh bone (femur) to the shin bone (tibia) and stabilizes the knee joint.
  • An ACL injury is caused by sports or fitness activities that exert pressure on the knee due to sudden jumping, stopping or changing direction movements.
  • An ACL injury is treated by physical therapy in relatively inactive patients, engaging in moderate exercise or playing sports that exert less pressure on the knees.
  • Surgical treatment of ACL is advised in cases of:
    • Athletes involved in sports that put continuous pressure on knees
    • If more than one ligament is damaged
    • If the meniscus of the knee joint is torn
    • Trauma or injury causing the knee to buckle during everyday routine
    • The young age of the patient
  • ACL reconstruction surgical procedure is performed under anaesthesia. Small incisions are made to insert instruments and tubes with a light and a camera to reach the joint space.
    • In this procedure damaged ligament is removed and replaced with a replacement graft of the tendon. 
    • The tendon graft is secured by screws and other devices drilled into the thigh and shin bone. This graft serves as a scaffold for the growth of the new ligament tissue.
  • Recovery time for ACL reconstruction surgery is 9 months and after 12 months post-surgery patients can return to sports or exercise routine.
